What the human evidence actually is

Grade: Human trials

Randomised controlled human trials exist. Not necessarily approved, and not necessarily positive.

Real and substantial, but not finished. Phase 2 in NEJM (2023) reported about 24% mean weight reduction at 48 weeks at the highest dose — the largest figure published for any anti-obesity drug at that stage. A separate phase 2 reported large reductions in liver fat. Phase 3 TRIUMPH trials are ongoing. It is not approved, and everything sold as 'retatrutide' online is unregulated material of unverified identity.

How it is supposed to work

Agonises GIP, GLP-1 and glucagon receptors. The glucagon arm is thought to add energy expenditure on top of appetite suppression.

Safety

Dose-dependent nausea and vomiting. Heart rate increases were seen. Because the glucagon receptor is involved, effects on glucose and liver are more complex than with GLP-1 alone. No long-term safety data exists.

Storage and handling

Lyophilised peptide is generally stored at 2–8 °C, or below −20 °C for long-term storage. Reconstituted in bacteriostatic water (0.9% benzyl alcohol) a solution is typically treated as usable for about 28 days refrigerated, versus roughly 24 hours for plain sterile water, which has no preservative. Add the water slowly down the vial wall and swirl — never shake, which shears peptide chains and foams the solution.

The arithmetic is the dangerous part
