What the human evidence actually is

Extensive. SURPASS and SURMOUNT programmes. SURMOUNT-1 produced roughly 20% mean body weight reduction at the top dose. SUMMIT showed benefit in HFpEF with obesity. SURMOUNT-OSA led to FDA approval for obstructive sleep apnea in 2024 — the first drug ever approved for it. SYNERGY-NASH reported high rates of steatohepatitis resolution.

How it is supposed to work

Single molecule agonising both the GIP and GLP-1 receptors, with greater weight effect than GLP-1 agonism alone.

Safety

Gastrointestinal effects dominate. Same rodent thyroid C-cell boxed warning. Pancreatitis and gallbladder events reported. Compounded 'tirzepatide' from gray-market suppliers has been the subject of repeated FDA warning letters.

Storage and handling

Lyophilised peptide is generally stored at 2–8 °C, or below −20 °C for long-term storage. Reconstituted in bacteriostatic water (0.9% benzyl alcohol) a solution is typically treated as usable for about 28 days refrigerated, versus roughly 24 hours for plain sterile water, which has no preservative. Add the water slowly down the vial wall and swirl — never shake, which shears peptide chains and foams the solution.
